FGMI provides services on a Private-Pay basis and is also an approved provider through Medical Assistance.
We are currently providing services in Chisago, Washington, Isanti, and Pine counties.
Services are available for people with physical disabilities, chronic diseases, behavioral diagnoses and mental illness.
