Our Vision
Individuals with special needs will have the ability to sustain their independence and grow as active, need-fulfilling contributors to their community.
Services are available for people with physical disabilities, chronic diseases, behavioral diagnoses and mental illness.
We are currently providing services in Chisago, Washington, Isanti, and Pine counties.
- Foundations Group is currently hiring PCA's. We have many different shifts available in several different areas. If you are interested in working as a Personal Care Attendant please call our office at 651-408-1433 and speak to Russ for more details.$9.50/hour
